[Muscle] iso7816-4 erase binary command

hi all,
i am using the erase binary command as mentioned by iso7816-4 protocol. 
i am getting sw1=6d and sw2=0. the iso reference doesnt contain anything 
abt this error. can someone please tell me why i am getting this error. 
also the iso reference doesnt have how to create a file. can someone 
tell me how to do that. i am using simputer and the smartcard controller 
that is attached with it (philips) and payflex 1k card.
